I know in reality sex feels better without, so the fantasy should also be without.

The only times I've used condoms is when I've been with a women whose sexual history I know nothing about (aka: one night stands).
In the case of porn films the actors sexual history should be right up-to-date to the time of the shoot, so when they start f**king there are no worries.
