I usually only beat them once then move on, except for:
Resident Evil Director's Cut (PS1) +- 6 times
GTAIII (PS2) only the story 4 times
GTA: vice city only the story 3 times, 100% completion once and all I got was a stupid T-shirt
Ace Combat 4 (PS2) about 15 to 20 times
Mercenaries: Playground of Destruction (PS2) at least 3 times with each character, so at least 9 times.

I've finished many other games at least once but too many to list. And just finished Mercenaries: World in Flames (PS3) for the first time.

I've beaten Gears of War(Original) 15-20 times. Mostly on co-op. I loved that game hehe. Always atleast on Hardcore Difficulty.
I beat Fable(1) 4 times. Last time on The Lost Chapters.
Mercenaries(1) 3 times.
Guitar Hero(3) 7 times. Make new bands and beat it on medium/hard/expert.
Army of Two 4 Times. Once on normal, three times on pro.
Hitman: Blood Money 5 times.
The first Super Mario game for NES atleast 5 times.
Gears of War(2) three times so far.
I could go for awhile but this list is good enough.
