I understand that existing US case law gives very effective free license to parody, which is why one has always seen so very many feature-length porn films parodying mainstream cinematic hits. Last Exit to Brooklyn was a very dark film, so I think an especially effective porn parody might frequently make irreverent light of the various on-screen tragedies, if only by simply "breaking the fourth wall" to make smart-ass remarks directly to the audience.
The original film made enough of an impression upon its release that all of four years later an episode of The Simpsons made reference to the movie via its title, Last Exit to Springfield. However, I could find no evidence of any attempt to make a live-action hard-core parody. One online article about this 60's-cult-novel-based work writes: "The film version received excellent reviews and won a few critics' awards for Leigh's portrayal of Tralala, though its limited distribution and downbeat subject matter prevented it from becoming a commercial success." Oh yeah, gay guys also have non-trivial storylines; I suppose that with sufficient artistic license, one could rework the original plot to involve your character with them as well - if you like.
One of the problems with a gang-bang scene is the expense of casting so many actors. While it would not eliminate the cost of STI testing, the use of amateurs and newbies might help. Be advised however, it is well-known that without the use of something like Viagara, traditionally few men are "up to" the challenge of staying hard in front of an entire film crew. Perhaps the limited demands of a gang-bang scene make this less of an issue. I guess the famous Annabelle Chong film proved that quite a lot is possible if one tries.
Fortunately, Last Exit to Brooklyn takes place in very seedy settings, and it would be rather easy to shoot in a suitable place for a very small location fee.

Tonight I read about your increasing involvement with anal stunts.

Call me an unwanted nanny, but it is penny wise and pound foolish not to get professional medical input on extreme stunts. Don't trust the bullshit artists in the industry or some guarantee written in broken English on some goods imported from Bangkok. There are even veteran women in the porn business who are RNs. Surely they could give you professional advice and instruction for a modest fee. (Or did they already cover porn stunts in high school Health Ed?) I hope I am just a worry wart and you have already prudently consulted medical professionals. STIs are not the only way someone in the porn business can land up VERY DEAD far too soon.
