It's a fake cumshot..it's from an old series called Casey's Cumshots, which are still floating around. Think of it as the predecessor to the awful cumshotsurprise site.

The scene is from Deep Throat This #24. The best I could find for you was a short trailer for the movie which includes a brief clip of what you're talking about. If I find more I'll post it here for you.
