I would say by birth. For example, I wouldnt count Shyla Stylez as American since she was born in Canada.

By any of your criteria Julia Ann! If you go by general public's awareness, it's probably Jenna Jameson, OR Ron Jeremy! 
I'm not a huge fan of hers but IMO today, Sasha Grey. She even played lead in that mainstream artsy movie by Soderbergh.
I agree. Some are going to disagree I suppose because of their personal taste but objectively she has to be the top today.

There isn't any real top porn star right now. The industry is rather aimless. Nobody sticks out as a clear favorite or is even close. There isn't even really a handful of people that stick well above everybody else. At bests there are a grouping of people hanging out at a little above average. A lot of them aren't very good looking or they are getting old. Many of the ones that are good looking don't put out that much work, or are living off of their past reputation. To be blunt, a lot of them don't have a lot of class either. I wouldn't be surprised if a great many casual porn watchers didn't know who they are. It doesn't help that the pay is getting less over the past few years, and a lot of porn fans expect stupid act and a bunch of other degrading gimmicky things the top starts of the past, and potential great people today, wouldn't even think of doing, and rightly so. It also doesn’t help that no producers really care about promotion or making their people into stars anymore. They more or less want the quick buck out of them before they toss them aside and go for somebody else. The performers don’t help the situation any. Too many of them don’t realize to have the best chance of making it big you have to be professional and work at it like a job to be taken seriously. Too many people think sitting there and looking pretty are enough when it’s not.
If porn could get it's act together in the near future, a unlikely thing I know, there is definitely a vacuum where somebody that's great and a real superstar could easily rise to the top because they have little competition.

I don't know D-R...just because the landscape of porn has changed doesn't me it's impossible to judge who is the best, most attractive and most popular porn star.
There is far more gonzo type porn nowadays.... maybe gonzo is more popular and as such maybe porn stars gravitate to it more because of that.
When there were less above average performers it was easy for the above average performers to stand out. That doesn't mean there are non now...
There is bad porn nowadays to be sure...the question is, is there good porn still being made? I don't think anyone would say good porn is not being made. There is IMO so much porn..then still so much good porn.it is hard for women to distinguish themselves...even as such there are some... Sasha Grey is clearly one IMO.
Plus she will get some attention playing herself as Vinny Chase's girlfriend on the new season of HBO's Entourage (starting tonight?)

Also, while Jenna Jameson was collecting all the credit and name recognition during her time...Jewel Denyle was the one (American) making the best porn probably in the modern era.
Gonzo is made because it's cheap and anybody can produce it. You get what you pay for. While at first it probably seemed like a brilliant idea to the people in the industry and a lot of the fans it just ended up being a curse in disguise that has slowly dragged things down to the lowest common denominator. It might have taken the better part of a decade but the negative effects it brought about for the quick buck are now being felt in full.
There isn't more above average performers now. While porn of the past sure had some people that didn't look that good performing in it, and had it's share of drugged out people or naive young women that shouldn't have entered the industry, it had a lot of memorable beautiful people and most of the top people now couldn't hold a candle to those people 15 or 20 years ago, not to mention how they would fare against the best people back then. Most of the people in it at that time at least looked decent at least and a lot of them were incredibly hot. They also brought much more passion and eroticism to their performances. A good portion of them could also act, at least good enough to make the scene work and stimulate feeling in people. Pretty much everybody now acts like they don't give a damn, probably because they don't. Maybe part of the reason was back then at least in a little way some of the producers, directors, and performers saw it at least a little bit as a form or art, or for some other reason seemed to care more about their work even if they didn't. Maybe some people cared because they could actually get to a little level of stardom, which is almost impossible now. Now we get either one of two things in most productions. Either they just mail their performance in or they act so blatantly fake and contrived it's a turn off. There is very little now that even seems like a semi-realistic situation to stir ones fantasy. People that act the part of practical whores sitting on a couch somewhere isn't that great of a thing. It almost might just as well be a inanimate sex doll sitting there at that point.
As far as people like like Sasha Grey and Jenna Haze go unlike the stars in the past nobody but the most fanatical of porn fans are going to remember them 15 or even 10 years from now, and if they are what are considered the top stars it's just that much of an indication of how much the industry is in trouble.
