Looks like a Bra mark to me... not a surgery scar! I used to date an F Cup so I know that mark... takes a lot of work to hold those babies in!

It's hard to tell from the shot. I would say she's pregnant with significant "perkiness" along with some photoshop. Can't tell for sure though, there is significant fidelity loss, likely because of the source camera.
