It looks pretty clear to me that it is done surgically. Perhaps laser work.
They're both tattoos. The artist just used a pigment that matched the areola.

you can tell in the 2nd set that where the actual nipple ends and the tattoo or body modification begins.. it's not a naturally occurring feature if that's what you're unclear on..they're very cute... i'm not sure i'd want heart shaped nips forever though lol

Miss Bunny seems to have similar ones, but not as strongly outlined.
If there's a way to make those with less permanent means than tattoos that would still approximate the effect well in terms of color and texture, I think I would want to see more of that.
