Hardest level you've ever played.

Any Level past 16 in Elder Scrolls: Oblivion. Once you hit level 16 all the enemies just have their way with you. (Level as in character level. Get it?!)

But seriously. You want hard. Try playing Elder Scrolls II: The Arena. My God what a hard game. Made me cry. Like Legit Cry.
 
try to get the achievement in Limbo where you never die
that and the mile high club achievement (cod4, veteran on the epilogue)

i also find games that have a huge asian market are geared to have a much difficulter setting type. their hardest is usually waaay harder than most games' (ex: marvel vs capcom 3 on hardest vs any halo on mythic)
 
Sector 2 in Project X on Amiga. Always died halfway through the level. Only 6 sectors in the entire game, but I didn't get further than that. But that's nothing..

Almost every screen in I Wanna Be The Guy is pure and absolute platform hell.

The entire game of Demon's Souls. It is ball crushingly hard and has no learning curve. You either rock right from the bat, or get crapped on for a long time.
 
The level in Battletoads where you're flying on those hover-bikes at an insanely fast speed. Still have yet to pass it

The last level in Mike Tyson's Punchout. I still cant beat him.
Beating the Forgotten One in Castlevania Lament of Innoncence on 'Crazy' difficulty. It took me nearly 2 weeks just to beat him.
 
Halo: Reach - Exodus on Heroic difficulty. man, it's so frustrating to run around surrounded by enemies that can almost kill you on the instant.

Trying to beat the bonus level on Call of Duty Modern Warfare (the passenger jet scene) on the hardest difficulty setting. I'm always a second or two too late. :(

I haven't played that many games in my life but the Hell levels of Doom 3 were pretty tough to play without God mode on. There were so many monsters and not enough fucking health and ammo to fight off the demons. We need more health and ammo in Hell. And it was so fucking dark and impossible to see two feet in front of you.
 
I know its not the hardest level, but Socom 4 - Mission 12 gave me a shit fit. Any level that requires stealth in general is hard for me.
 
Level 6 of Ghosts N Goblins also The DLC in Left 4 Dead where you keep fighting endless tanks until you complete the objective.
 
God of War 1,think it was called the God challenge.It was the second to last level where you fight all those medusa's and undead soldiers. That level had me button mashing like I was crazy.

The final tower or whatever of Viking. The whole game was fairly easy. Then I get to the top of the last castle, tower whatever the fuck it's called and I can't do shit but die constantly. Fucking impossible. Pissed me off and I had to walk away forever without ever beating that fuckin game.

For the EMPEROR!!
Halo Reach. The level where you land on and infiltrate the Covenant corvette (Legendary mode). Especially when you go back to the hangar after the bridge. Those Elites were tough.
 
A level I found hard was in MW2 (on the hardest difficulty) when you're storming the front lawn of the white house. That's fucking intese. It took me a little bit to figure out to get on the turret and take out the turrets in the whitehouse. I was just running blind through the battlefield for the first few times I did it. It's amazing in the sense that on the easier difficulties, I didnt have to do that.

I would have to say getting the mile high achievement on mw1 was hard as hell. Had to be precise. The only other level of any game that was like that was probably the nuclear launch site on same game on veterin difficulty. That fucking took me days.
