No, i think it was an amateur shot made by Chico Wang (r.i.p.) shortly before Madison quitted porn. The fat guy you mention was probably the minion.
As you say, it was indeed sort of a joke since it was intended as some "promo ad for safe sex"

This is the one rare video about her i cannot manage to find.
