Depends on the circumstances.
If Im trying to finish a story so nobody can spoil it for me, or if I'm trying to do something like find hidden items I will do it on easy.

If I am in no hurry I will do Medium.

If I want achievements or am very bored I will do hard.

Normal and/or hard. If I know a game (played the previous game('s)), I'll probably opt for hard straight away, unless it's know for it's difficulty off course.

First-person shooters on the computer I always play on the hardest difficulty. Ever since DOOM.

Other types of games I'll usually play on whatever the default difficulty is.
