If the tits are well done, no one can tell the difference, especially horny guys. I know this from experience. I have arguments EVERY nite in my chatroom with guys who insist that my tits are real. I think that women could tell the difference, especially if they know what to look for, but men usually can't. I think the only way a man can tell that they're fake is if the tits are abnormally huge, or the scar/ugly ripples give them away or they suffer from "beach ball" syndrome. But if they're well done, you really can't tell. Mine are brand new (2 months post surgery) and everyone thinks they're natural. I love it.

Women just know boobs. We have them all our life, we've all seen our gfs boobs, and we know how they're supposed to look, feel, move etc. Seeing them in porn, having them pushed in your face by strippers and even squeezing them in bed still doesn't give guys that kind of knowledge lol. I have dozens of women I'm friends with who have had boob jobs, and their significant others STILL don't know that their gf/wife has fake tits and they sleep and live with them! Makes me laugh lol.

Taking all implants doen as a whole I cold probably tell whether breast are real or fake about 99.5% of the time if the breast are exposed and I get more than a few angles in pictures to look at them. There's maybe one out of around 500 that are done so well that I have to really question it. Usually it's not that hard though.
